There are 4 sections and answering all questions in each section is mandatory - (A) Employee Volunteering in Your Company; (B) Key Volunteer Engagement Programmes Undertaken in between January 2022 and August 2023; (C) Company Profile and (D) Supporting Documents & Declaration.
Kindly review the information you have entered for accuracy. Please also note:
- This form will take a minimum of 30-40 minutes to complete
- Read the declaration carefully (last page)
- Once submitted your application cannot be edited
- Incomplete application forms will not be considered
- If any information is found misleading, the jury reserves the right to reject an application at any stage
